Our Mission and Vision

Our Mission at Mustard Seed Community Health is to provide high quality, holistic, integrated healthcare to those in need, in an environment that dignifies and empowers the whole person.


Our Vision? We will live in a healthy community where all have access to quality healthcare, and individuals, families, faith communities, service organizations, foundations, and businesses partner together for the common good.


Without Mustard Seed in the community, many Cottage Grove residents would go without medical care, or would see a doctor only when their health problems became an emergency. As an integral part of the community, we help people get healthy–and stay healthy. 

 

Everyone deserves to enjoy a full, healthy life with access to high-quality care – without the all-to-usual burden of healthcare debt. At Mustard Seed, our patients find friendly, low-cost or no-cost healthcare resources close to home. Whether you live in our Cottage Grove neighborhood or in another part of Guilford County, Mustard Seed’s clinic family is here to help you reach your health goals. 

Last year, patients used Mustard Seed health resources to reach their goals: 

608 Patients

437 uninsured patients and 171 underinsured patients received primary health care from Mustard Seed

Diabetes Managed

80% of diabetes patients successfully lowered their A1C

Blood Pressure Lowered

71% of hypertension patients successfully lowered their blood pressure

Holistic Care

1,310 neighbors attended clinic visits and received the holistic primary care they needed, in Spanish and English

Community Health

2,936 neighbors received community health visits and connected with the resources they needed, in Spanish and English

Health & Resilience

25 families in the Resilient Inclusive Program received healthy food deliveries, nutrition classes, and stress management help

Why Are We Called Mustard Seed?

This is the Bible’s parable of the Mustard Seed: “The kingdom of heaven is like a mustard seed, which a man took and planted in his field. Though it is the smallest of all seeds, yet when it grows, it is the largest of garden plants and becomes a tree, so that the birds come and perch in its branches.” (Matthew 13:31–32). 


To us, the mustard seed represents the impact we hope our clinic has on our community: From small things, great things can grow!
